Job Summary:At Prisma Health, we are seeking a compassionate and dedicated registered nurse to join our hospice care team. As a registered nurse in this role, you will have the opportunity to make a meaningful impact on the lives of patients and their families during some of their most challenging times.About the Role:This is a unique opportunity to utilize your nursing skills and experience to provide high-quality, patient-centered care to individuals with terminal illnesses. You will work closely with a multidisciplinary team to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.Responsibilities:

• Assess patient physiological, psychosocial, developmental, environmental needs, and the families' need for supportive care.
• Develop and implement individualized nursing and CNA plans of care, seeking input from patients and families as needed.
• Provide education to patients and families on end-of-life care and support.
• Document all clinical findings, interventions, physician orders, changes in the plan of care, telephone calls, team conferences, and referrals.
• Initiate the plan of care at the time of death and notify all appropriate persons/agencies.
• Perform admission, evaluation visits, and consultative services as requested.Requirements:

• Associate degree in Nursing
• Valid RN license
