Hospice Care Specialist OpportunityPrisma Health is seeking a compassionate and skilled Registered Nurse to join our Hospice team as a Hospice Care Specialist. In this role, you will provide palliative care to terminally ill patients and supportive care to their families in a Hospice House setting.Key Responsibilities:

• Assess patient physiological, psychosocial, developmental, and environmental needs, and the families' need for supportive care.
• Develop an individualized nursing and CNA plan of care, seeking input from the patient and family.
• Implement appropriate nursing interventions with thorough and timely documentation in the patient's chart.
• Evaluate the effectiveness of specific interventions and the resulting movement toward goals.
• Provide bereavement support to families through follow-up visits, supporting the bereavement plan of care, and attending funeral services whenever possible.Requirements:

• Associate's Degree in Nursing
• Valid driver's license and acceptable motor vehicle record
• Valid RN licensure
• BSN preferred
