Job Description:

Inspire health, serve with compassion, and be the difference. The hospice registered nurse provides hospice and palliative care to terminally ill patients and supportive care to their families in the Hospice House.

Responsibilities include:

• 
  
• Assessing patient physiological, psychosocial, developmental, and environmental needs, and the families' need for supportive care.
  
• Developing an individualized nursing and CNA plan of care, as needed.
  
• Providing education to patients and families, and monitoring their need for additional services.
  
• Overseeing CNA's performance of the Plan of Care.
  
• Evaluating the effectiveness of specific interventions and the resulting movement toward goals.
  
• Providing continued support for families during the bereavement process.
  
• Facilitating continuity of care with other team members and agencies.
  
• Communicating with physicians, other staff, and volunteers to coordinate and ensure quality care.
  
  
  Requirements include an Associate's Degree in Nursing, a valid driver's license, and a certificate of auto insurance. 2 years of acute care nursing experience is preferred. The position is bonus eligible.
